Since 1978 there has existed a therapeutic possibility to help the childless couples to have an own baby with the help of In vitro Fertilisation followed by Embryo Transfer.

. Generally, the In vitro fertilisation or the extracorporeal fertilisation is understood as joining of the egg and sperm cells (= fertilisation) outside of the body.
. In the case of this procedure, we remove the egg cells that have grown after a preliminary and controlled hormonal stimulation of the ovaries with the help of a follicular puncture and bring them together with the sperms of the man outside of the body, which means that the sperms penetrate the egg cells without further help. In about 50-70% of the cases, the egg cells can be fertilised this way. With the help of a plastic catheter, up to 3 embryos are transferred into the uterus. This procedure is called Embryo-Transfer (ET).
. This method is primarily applied in the case of a female fertility disorder.
. Due to a significant progress in the reproductive medicine, we can now help the couples who did not have any chances of a successful therapy before.
. The intracytoplasmic sperm injection (ICSI-Method) is considered a further milestone in the sterility therapy. With the help of this technique, a single sperm is injected directly into the egg cell, in order to achieve the fertilisation (in comparison to it, during the sexual intercourse about 200 million of sperms are released.)With the help of an intracytoplasmic sperm injection, fertilization rates of more then 60 % of all egg cells can also be achieved in patients with an extremely low sperm concentration (less then 10.000 sperms in the whole ejaculate). This method can also help the patients in whom no motile sperms in the ejaculate can be seen primarily.
. If no sperms at all can be found in the ejaculate because of epididymis diseases, the sperms can be taken from the epididymis and used for the intracytoplasmic sperm injection (Microsurgical Epididymal Sperm Aspiration, MESA). If no sperms can be found in the epididymis itself, then it is also possible to obtain the sperms directly from the testicles [(Testicular Sperm Extraction (TESE)].
